Free T4

Free T4 measures thyroid hormone that is not attached to proteins and helps make sense of TSH.

The thyroid’s feedback loop

TSH is a request from the pituitary. T4 is part of the thyroid’s response. Seeing the loop helps explain why the two results can move in opposite directions.

The thyroid’s feedback loop The pituitary sends a signal to the thyroid; thyroid hormone then feeds back to adjust that signal. A simplified loop with an intact pituitary–thyroid system. 1. The pituitary sends TSH

The pituitary is a small gland at the base of the brain. It releases TSH to stimulate the thyroid. When thyroid hormone is low, a working feedback system usually increases this request.

  2. The thyroid releases hormone

The thyroid responds by making and releasing thyroid hormones, mainly T4. The free T4 test measures the part not attached to blood proteins, giving context to the TSH signal.

  3. Thyroid hormone turns down the request

As the hormone signal rises, it reduces further TSH release. This is called negative feedback: the response limits the original request. It helps keep hormone production in balance.

What does it measure?

The thyroid produces mostly thyroxine, called T4. A small fraction circulates unbound, and tissues can convert T4 into the more active hormone T3. Free T4 testing is useful when assessing suspected thyroid disease or monitoring some forms of treatment. [1]

Before the test

Sample
A blood sample from a vein.
Preparation
Usually no special preparation is needed. Tell the team about thyroid medicine, supplements, and pregnancy.
[1]

What can the result mean?

Low free T4 with high TSH usually points toward an underactive thyroid. High free T4 with low TSH may indicate an overactive thyroid or excess replacement hormone. Low free T4 with a low or ordinary TSH needs a different assessment, sometimes involving the pituitary gland. [1]

When is it useful, and how is it read?

Free T4 adds meaning to an abnormal TSH and is especially important if pituitary disease is possible. A low free T4 with high TSH differs from a low free T4 without the expected TSH rise. Pregnancy and thyroid treatment also require interpretation tailored to the situation. [1] [4]

Low free T4 with high TSH differs from low free T4 with a low or ordinary TSH. The pair helps frame the clinical question. [1]

What can affect it?

Laboratory methods, pregnancy, serious illness, and medicines can affect the result. Biotin may interfere with some assays. If you take thyroid medication, the time between the dose and blood draw matters for comparable follow-up; follow your clinician's plan rather than skipping treatment independently. [1]

An interesting fact

Free and total T4 are different measurements

Total T4 includes hormone attached to carrier proteins; free T4 measures the unbound portion. Changes in carrier proteins can therefore alter total T4 without producing an equivalent change in thyroid function. [1] [4]

Questions you might have

What does “free” mean in free T4?

It means T4 not attached to blood proteins. Total T4 includes both the bound and free portions. [1]

Can free T4 be interpreted without TSH?

Usually the pair is more helpful. A low T4 with high TSH suggests a different pattern from low T4 with low or ordinary TSH. [1]

What comes next?

Read free T4 and TSH as a pair, with your symptoms and previous results. Unexpected combinations may need repeat testing or a laboratory-method check before treatment changes. A single free T4 value cannot diagnose the cause of fatigue or explain every change in weight. [1]

A question to take with you

Do free T4 and TSH tell a consistent story, or should we check timing and the test method?

The reference interval and units on your report are part of understanding your result. Understand units, reference intervals, and treatment targets.
