What does it measure?
Proteins and sometimes cells collect into the shape of a kidney’s small tubes. The laboratory looks for these structures in urine sediment and describes the type. “Casts present” is less informative than knowing whether they are hyaline, red-cell, white-cell, or another type.
Before the test
- Sample
- A urine sample examined under a microscope. Follow the collection instructions.
- Details
- Keep the cast type and quantity on the report; “casts present” is incomplete information.
What can the result mean?
A few hyaline casts may appear after exercise or dehydration. Cellular casts can provide stronger clues to a process within the kidneys. Different cast types have different meanings; finding any cast does not automatically mean kidney failure. [1]
When is it useful, and how is it read?
Read the named cast type with kidney filtration, urine protein, blood, symptoms, and recent illness. “Casts present” without a type is too broad to distinguish an exercise-related finding from an active kidney problem. [1]
What can affect it?
Casts can break down if a sample stands too long. Urine concentration, recent strenuous activity, and the quality of microscopy influence what is found. Their interpretation should include urine protein and kidney blood tests. [1]
Casts carry a clue about where they formed
Casts take shape inside the kidney’s tiny tubes. Their contents matter: a few simple hyaline casts can differ greatly from a cast containing red or white blood cells. [1]
Questions you might have
Are all casts abnormal?
No. Small numbers of hyaline casts can occur after exercise or dehydration; other types have different significance. [1]
Why is the type more useful than just the count?
Casts containing red cells, white cells, or other material point to different processes within the kidneys. [1]
What comes next?
Ask for the exact cast type and whether the finding was confirmed. Your clinician can decide if a fresh sample is enough or if kidney-focused assessment is needed, especially with swelling, blood in urine, or reduced urine output. [1]
What type of cast was found, and does it match my other kidney results?
The reference interval and units on your report are part of understanding your result. Understand units, reference intervals, and treatment targets.
