What does it measure?
HDL particles take part in moving cholesterol between tissues and the liver. The blood test measures the cholesterol they carry, rather than directly testing how well those particles work. This is why the familiar label good cholesterol is helpful only up to a point. [2]
Before the test
- Sample
- Blood for a lipid profile.
- Fasting
- Some lipid profiles can be taken without fasting. Follow the laboratory’s instructions for your order, especially if triglycerides are being rechecked.
What can the result mean?
Low HDL often appears alongside higher triglycerides and metabolic risk. But a very high HDL result is not a guarantee of protection. Original population studies found that the relationship is not simply higher is always better; an association also does not prove HDL itself caused an outcome. [1]
When is it useful, and how is it read?
HDL contributes to cardiovascular risk assessment alongside LDL, non-HDL cholesterol, triglycerides, and your history. A reassuring HDL result does not cancel an elevated LDL. Decisions should address overall risk rather than aiming to raise HDL in isolation. [2]
What can affect it?
Genetics, medicines, and metabolic health influence HDL. Its meaning depends on LDL, non-HDL cholesterol, blood pressure, smoking, and other risk factors. A high HDL should not cancel out attention to a high LDL or a strong family history. [2]
Myth: «The higher HDL goes, the better»
Extremely high HDL was linked with higher mortality in large observational cohorts. That does not prove HDL caused harm, but it challenges the idea that continually raising the number guarantees more protection. [1]
Questions you might have
Does very high HDL always mean extra protection?
No. Cohort studies found that extremely high HDL was not consistently reassuring. HDL needs the rest of the risk picture beside it. [4]
What comes next?
Focus the discussion on reducing overall cardiovascular risk. Regular movement and an eating pattern suited to you can support health without treating HDL as a score to maximize. Do not start alcohol or a supplement to raise it. Your clinician can explain which lipid measure is most useful for treatment decisions. [2]
How does my HDL change the overall assessment when LDL and my other risks are included?
The reference interval and units on your report are part of understanding your result. Understand units, reference intervals, and treatment targets.
