Cortisol (AM)

Morning cortisol helps answer selected adrenal questions; it is not a direct measure of everyday stress.

What does it measure?

Your adrenal glands make cortisol to support blood pressure, blood sugar regulation, and the response to illness. Levels usually rise around waking and fall later in the day. An AM test measures blood cortisol at a recorded morning time, usually as part of a specific clinical assessment. [1]

Before the test

Sample and time
A morning blood sample. Record collection time and your usual sleep schedule.
What to mention
Include steroid tablets, injections, inhalers, and creams in your medicine list.
[1]

What can the result mean?

A low morning result may raise concern about insufficient cortisol production, but often needs confirmation with a stimulation test. A higher result may reflect illness, stress, pregnancy, or changes in hormone-binding proteins. A single morning value is not a recommended stand-alone test for Cushing syndrome. [1] [2]

When is it useful, and how is it read?

A morning sample may help assess possible cortisol deficiency in the appropriate clinical setting. Suspected cortisol excess requires different validated tests. Symptoms, steroid exposure, sleep pattern, and sometimes further dynamic testing determine what the result can actually answer. [1] [2] [3] [5]

What can affect it?

Timing relative to sleep matters, especially with night shifts. Oral, injected, inhaled, and topical steroid medicines can affect the assessment. Estrogen-containing medicines may raise total cortisol through binding proteins. Tell your clinician about all of them and follow the agreed collection plan. [1]

An interesting fact

Cortisol normally follows a daily rhythm

For people who sleep at night, cortisol is usually higher around waking and lower later in the day. A morning result is one point on that rhythm, not a measurement of your stress level over the whole day. [4] [6]

Questions you might have

Can a stressful blood draw affect cortisol?

Stress and exercise can raise cortisol. This is one reason the collection conditions belong beside the number. [1]

Does a high morning value diagnose Cushing syndrome?

No. A single morning blood cortisol is not a recommended stand-alone screening test for Cushing syndrome. [2]

What comes next?

Your clinician may select repeat testing, ACTH, a stimulation test, or a different cortisol test depending on the question. Do not stop steroid treatment abruptly. Fatigue deserves careful assessment, but a cortisol number alone cannot explain all its possible causes. [1] [2]

A question to take with you

Was the timing appropriate for my sleep schedule and the adrenal condition being assessed?

The reference interval and units on your report are part of understanding your result. Understand units, reference intervals, and treatment targets.
