Alpha-Fetoprotein (AFP)

AFP is useful for specific liver or tumor assessments, not a general cancer check.

What does it measure?

Alpha-fetoprotein is produced in large amounts during fetal development and is usually much lower outside pregnancy. In adults, a clinician may use it with other tests to assess or monitor certain liver or germ-cell conditions. Pregnancy-related AFP testing has a separate purpose and interpretation.

Before the test

Sample
A blood sample from a vein.
Context
State whether you are pregnant and whether the test is for liver assessment or known tumour follow-up.
[1]

What can the result mean?

A raised AFP can occur with some cancers, but also with noncancerous liver disease or pregnancy. A normal result cannot exclude cancer. AFP alone neither diagnoses a tumor nor works as a broad screening test for healthy people. [1]

When is it useful, and how is it read?

Outside pregnancy, read AFP within a defined liver or germ-cell assessment and alongside imaging when indicated. In high-risk liver surveillance or known cancer follow-up, its trend can add information; an isolated result is not a general cancer check. [1]

What can affect it?

The reason for testing, pregnancy status, liver history, and previous results are essential. Some high-risk liver surveillance plans include AFP together with imaging; this is different from offering it as a general wellness test. [1] [2]

An interesting fact

AFP has a different meaning during pregnancy

Pregnancy-related AFP testing and tumour-marker testing ask different questions. Pregnancy stage or the reason for tumour follow-up must be known before a value can be interpreted. [1]

Questions you might have

Is this the same purpose as AFP testing in pregnancy?

No. Pregnancy screening and tumour-marker testing ask different questions and use different interpretation. [1]

Can a normal AFP rule out cancer?

No. Some cancers do not raise AFP, and noncancerous liver conditions can raise it. It is not a universal cancer screen. [1]

What comes next?

Ask what specific decision AFP is meant to inform. Your clinician may compare trends, liver tests, and imaging rather than react to one result. If there is no clear indication, reconsider whether the test adds useful information. [1]

Why is AFP appropriate for me, and what other evidence is needed to interpret the result?
